To request a copy of your medical records to be sent to another provider, please fill out our medical records release form available at the clinic or on our website. Ensure you provide the name and contact information of the receiving provider. Once completed, submit the form to our front desk or email it to our records department. We will process your request and send the records directly to the designated provider.
Everyone’s needs are different, so have a chat to your dentist about how often you need to have your teeth checked by them based on the condition of your mouth, teeth and gums. It’s recommended that children see their dentist at least once a year.

To ensure that the person you designate has access to your medical records, you must complete a medical records release form specifying the individual's name and relationship to you. This form can be obtained at our clinic or downloaded from our website. After submitting the form, we will grant the designated person access to your records in accordance with your request.
